AMSSM LIBRARY OF SPORTS ULTRASOUND PATHOLOGY


Objective
The Sports Ultrasound Committee looks to create an educational resource directed at residents, fellows, and other learners to review both common and unusual pathology within sports ultrasound using case examples. In exploring the library of sports ultrasound pathology, we intend for learners to gain an appreciation for the nuances of ultrasound imaging and increased confidence in recognition of abnormal findings.

Submission Guidelines

  • Current sports medicine fellows are particularly encouraged to submit, with faculty guidance.
  • If a trainee is preparing the submission, the content should be reviewed with a local attending to verify ultrasound image quality and image interpretation accuracy.
    • Following submission, a secondary review will be performed by an AMSSM subcommittee. Edits may be suggested or if images are of low quality, the case may be rejected. Thereafter, the accepted content will be posted and available for member review.
  • All content must be de-identified prior to submission.
  • High quality images and/or videos should be submitted. Low quality images will not be accepted.

Quadriceps Myositis Ossificans
A 16-year-old male high school football athlete presented after right thigh injury sustained during football practice 2 weeks prior with continued anterior thigh pain with ambulation, knee flexion, and with a decreased range of motion.
 
Distal Biceps Tendon Rupture
An ambidextrous 60-year-old male presents with acute distal right forearm pain and bruising following an injury as he was getting pulled up by a boat to water ski. The patient felt a popping sensation and immediately experienced pain following the injury. He complained of weakness with forearm flexion and supination at the time of examination.
 
Proximal Patellar Tendinopathy
A 30-year-old male multi-sport athlete presented with a 6-month history of right anterior knee pain. There was a sudden onset of pain when loading to jump while playing volleyball. Pain was described sharp, had been persistent since onset, and worsened with explosive movements. Exam was notable for intact extensor mechanism, although weak and painful with knee extension, and tenderness to palpation of proximal patellar tendon.
 
Distal Clavicle Osteolysis
A physically active 30-year-old male presented to sports medicine clinic for left anterior shoulder pain that was worse during and after performing push-ups. A positive Scarf test was noted.
 
Retrocalcaneal Bursitis
A 21-year-old female presented with non-traumatic right heel pain for approximately 3 months. Exam was notable for tenderness on calcaneal squeeze and tenderness to palpation deep to the achilles tendon. Pain was worse with passive and active dorsiflexion of the ankle.
 
Pes Anserine Bursitis
A 48-year-old previously-sedentary male presented with 3 weeks of right anteromedial knee pain with running. On examination of his right knee, there was full ROM, normal ligamentous exam, no joint line tenderness, but positive tenderness to palpation over pes anserinus.
 
Proximal Adductor Tendon and Muscle Tears
A 32-year-old male soccer player presented with acute right groin pain while kicking a ball 3 days prior. Exam was notable for tenderness to palpation over proximal right adductor longus and pubic tubercle, pain with resisted adduction with hips in both neutral and flexed positions, and full ROM.
 
Morel-Lavallee Lesion of the Lateral Hip
A 38-year-old female runner presented with diffuse right hip pain for 3 days. She fell on her right side at mile 10 of a 50 mile race and slid along the trail. She had immediate onset pain, and noted swelling thereafter. Exam showed visible swelling and an abrasion. Full ROM and strength of the hip were noted without deficits.
 
Baker's Cyst
A 62-year-old female presented with one month of atraumatic right posterior knee swelling. Her exam was normal except for noted mild popliteal fossa swelling.
 
Elbow UCL Tear
A 16-year-old right-hand-dominant male baseball pitcher presented with acute onset right medial elbow pain while pitching during a baseball game. Exam of his right elbow displayed tenderness to palpation of the medial elbow immediately distal to the medial epicondyle, and pain and laxity with valgus stress test and milking maneuver.
