His PHQ-9 and GAD-7 were scored zero. C3, which is the Cleveland Clinic Concussion (C3) mobile app, is a tool used for assessing concussion symptoms and compares assessments of balance, reaction time, memory and vision after injury to baseline and normative data. His C3 testing was within averages of normative data except for a symptom of neck pain.
His Balance Error Scoring System (BESS) had a total of 7 errors. Immediate Post-Concussion Assessment and Cognitive Testing (IMPACT) testing was at or above baseline.
Because of the loss of consciousness and ocular abnormality, an MRI brain without contrast was obtained three days after the outpatient visit, which was 6 days after injury. It revealed no acute or chronic findings.
Consultations:
The patient was referred to a system-based optometrist who specializes in ocular dysfunction after concussions. Her exam, which included visual acuity, tonometry, refraction, accommodation, external exam, slit lamp and fundus exam, was normal. Near point of convergence was reduced to seven centimeters. Since the patient had no vision changes with testing and was otherwise asymptomatic, the optometrist diagnosed his lateral gaze defect as most likely chronic or congenital. She felt he had compensated well for the abnormality throughout his lifespan and thus had no clear ocular symptoms.
