Ultrasound-guided glenohumeral joint injection was planned for suspected adhesive capsulitis. During the pre-injection scan, the sonographer noticed abnormalities of the humeral head including cystic changes, cortical bone thinning, and posterior acoustic enhancement Case Photo #1 . Injection was aborted. Shoulder radiographs were repeated and showed a lytic lesion in the humeral head Case Photo #2 . Urgent magnetic resonance imaging revealed a soft tissue mass in the posterior humeral head with extension into the glenohumeral joint and subacromial subdeltoid bursa Case Photo #3 .

Consultations:
The patient was referred to interventional radiology for biopsy of the humeral head mass.
