Right shoulder MR arthrogram obtained three months after symptom onset demonstrated rotator cuff tendinopathy without a full-thickness tear and early infraspinatus atrophy. MRI performed at five months revealed supraspinatus and infraspinatus tendinosis with bursal-sided fraying, early fatty atrophy of the infraspinatus, posterior labral fraying, and convex remodeling of the posterior glenoid.

Electromyography confirmed suprascapular mononeuropathy distal to the branch supplying the supraspinatus. Right shoulder radiographs obtained at presentation to our clinic, six months after symptom onset, were unremarkable.
