An x-ray of the right shoulder was negative and an MRI of the right shoulder showed no evidence of a rotator cuff tear. X-rays of the cervical spine showed straightening of the cervical lordosis and concern for a C5 lamina fracture. A CT of the cervical spine confirmed a fracture of the right C5 lamina extending into the right C5 facet Case Photo #1 . There was associated mild widening and mild unilateral subluxation of the right C4-C6 facet joints. An MRI of the cervical spine showed a small C5-C6 posterior disc osteophyte complex with moderate bilateral neuroforaminal narrowing. At C6-C7 there was a small central disc protrusion with no significant spinal canal narrowing. The nerve conduction studies revealed electrophysiologic evidence consistent with an active right C5 radiculopathy. There was no evidence of a focal entrapment neuropathy, right ulnar sensory response was normal and right median to radial sensory comparison study was normal.
