Patient Presentation:
Chronic bilateral exertional lower leg pain

History:
A 16-year-old elite soccer player was referred by her pediatrician to the orthopedic office for consultation of chronic bilateral exertional lower leg pain. The pain was non-bony, diffuse, described as tightness around the calves. Her pain was worse with exertion and completely resolved with rest. Her review of systems was positive for excessive fatigue with exercise and feeling abnormal cold.

Physical Exam:
Inspection: no obvious deformity, atrophy, swelling, or fascial herniation noted in bilateral legs
Palpation: no bony or muscle tenderness to palpation in the bilateral legs
Range of Motion: full range of motion of her knees and ankles
Motor: 5/5 strength in all motions
Maneuvers: No foot drop with normal reflexes bilaterally
