Broad Differential Diagnosis:
Both intra-articular (glenohumeral) and extra-articular etiologies can cause anterior shoulder pain. Additionally referred pain can arise from the neck or visceral sources. Intra-articular causes most commonly include glenohumeral osteoarthritis, labral injury, or shoulder instability. Extra-articular causes include impingement syndromes, rotator cuff muscle and tendon injuries, and acromioclavicular joint dysfunction. Referred pain could be due to cervical radiculopathy, brachial plexus injury, mononeuropathy, or from cardiac, pulmonary, or gastrointestinal sources.
