Adductor Tear

Authors, Editors, Case Information Author: Garry Ho, MD, FAMSSM, FAC Affiliation: OrthoVirginia Co Author(s): None Senior Editor: Raul Rosario-Concepcion, MD Clinical Vignette: 32 year-old male soccer player presented with acute right groin pain while kicking a ball 3 days prior. Exam with tenderness to palpation over proximal right adductor longus and pubic tubercle, pain with resisted adduction with hips in both neutral and flexed positions, full ROM, and distally neurovascularly intact. Type of Probe Used: 4-12 MHz linear transducer. Adductor origin at pubis - unlabeled Left and right long-axis views of the adductor longus and brevis origin at the pubic tubercle and body. Adductor origin at pubis - labeled Left and right long-axis views of the adductor longus and brevis origin at the pubic tubercle and body. Interpretation Image Interpretation: High grade tear of right proximal adductors at pubic tubercle and body, involving at least 75% of adductor longus tendon, fibrocartilage, and muscle, extending to anterior third of adductor brevis. Normal appearing origin of left adductor longus and brevis and pubic tubercle and body. Teaching Pearl: The adductor longus is most commonly injured during sporting activity. The relative length, greater tendon to muscle ratio, and weaker pubic attachment may make the adductor longus more susceptible to injury.
