Lab Studies:
White blood cells 3.2, platelets 41, absolute neutrophil count 0.7.

Other Studies:
A 3-view radiograph of the right shoulder showed mild acromioclavicular joint osteoarthritis without evidence of fracture.

An MRI of the right shoulder showed a 2-centimeter x 3-centimeter bone marrow signal abnormality in the anterior humeral head. There was acromioclavicular joint osteoarthritis with trace subacromial subdeltoid bursitis, a degenerative labral tear with a small glenohumeral effusion, and mild supraspinatus and infraspinatus tendinosis.

Peripheral blood flow cytometry showed 2% kappa-restricted B-cells with typical features of classical type hairy cell leukemia. This was confirmed by bone marrow biopsy.

Consultations:
The patient was referred to an orthopedic oncologist.
