A Complicated Acl Recovery In A Professional Soccer Athlete - Page #3

Lab Studies:
Not indicated

Other Studies:
Punch Biopsy Right Anterior Thigh Lesion:
Malignant melanoma, superficial spreading type. Breslow depth: 0.6 mm. Ulceration: absent. Pathologic stage: pT1a (Stage IA). Sentinel lymph node biopsy: not performed (not indicated for T1a melanoma per current National Comprehensive Cancer Network guidelines).

MRI Right Knee without contrast:
ACL reconstruction with intact graft. Lateral meniscus repair with no discrete MRI findings or a re-tear. Focal extensive bone marrow edema in the inferior pole of the patella. Although this is in the location of the BTB harvest site, the degree of edema is more than one would expect 7 months postoperative. Extensive edema of the infrapatellar fat pad. Case Photo #2 , Case Photo #3

Consultations:
Following biopsy confirmation of malignant melanoma, Dermatology was consulted.
Wide local excision via Mohs micrographic surgery was recommended. Given the
diagnosis of melanoma, a tumor known to spread via lymphatic and hematogenous
routes, the multidisciplinary team was also asked to evaluate mucosal and ocular sites,
as primary melanoma can arise from uveal, oral mucosal, and gynecologic mucosa.
Consultations with Obstetrics/Gynecology, Ophthalmology, and Dentistry were obtained
accordingly.
Orthopedic Surgery was consulted regarding knee management options. Physical
Therapy was engaged specifically to address the intensity of the ACL rehabilitation
program and to evaluate the risks and benefits of continuing Blood Flow Restriction
(BFR) therapy, which had been incorporated into her rehabilitation protocol prior to the
melanoma diagnosis.
