EKG: normal sinus rhythm. Unremarkable.

A ZIO XT patch was placed on the patient, and he was advised to keep a diary of symptoms. Results obtained the following weekend noted multiple episodes of narrow and wide complex tachycardia at rates of 160 to 180 beats per minute lasting 50 to 120 seconds, all of which occurred while the patient was sitting and eating.

The patient was advised to go to the emergency room for further evaluation, admission, and cardiac workup. CBC: unremarkable. CMP: mildly elevated creatinine at 1.35 (near baseline). Troponin: negative. TSH and glucose: within normal limits. CXR: no acute process. EKG: normal sinus rhythm with no ST elevation. TTE: unremarkable, LVEF = 66%.
