Limited diagnostic US at office visit: Fully ruptured quadriceps tendon proximal to patella.
MRI:

Complete avulsion of the rectus femoris contribution of the quadriceps tendon from the patellar attachment site with tendon retraction of 5 cm and surrounding hemorrhage and edema.
