Lab Studies: None Other Studies: MRI of the left elbow showed multi-structural injury including complete tear of the common flexor tendon Case Photo #1 , tear of the ulnar collateral ligament complex with essentially complete tearing of the lateral radial collateral ligament (LRCL) and lateral ulnar collateral ligament (LUCL) Case Photo #1 , complete radial collateral ligament tear Case Photo #2 , and persistent posterior lateral subluxation of the radius and ulna relative to the humerus Case Photo #3 . There was also partial dislocation of the ulnar nerve from the cubital tunnel. No coronoid or radial head fracture was noted. This constellation of findings was consistent with acute elbow dislocation and persistent posterior subluxation of the radial head. Photo 1: Coronal T2 fat saturated image showed a complete tear of the common flexor tendon (thick arrow) with irregular, slightly retracted appearance of the tendon fibers. Additionally, there was a complete tear of the proximal ulnar collateral ligament near the humeral attachment (thin arrow). Case Photo #1 Photo 2: Coronal T2 fat saturated image showed separation of the proximal attachment of the radial collateral ligament from the humerus (arrow). Case Photo #2 Photo 3: Sagittal T2 fat saturated image showed posterior subluxation of the radial head relative to the capitellum with increased T2 signal in the posterior capitellum (arrow) consistent with bone contusion.
