- Muscle Biopsy (1995): Rare regenerating fibers are identified, However, no definitive etiology can be determined.
- EMG Left Lower Extremity: Left Lumbosacral Polyradiculopathy.
- EMG Left Upper Extremity: Left C7 Cervical Radiculopathy
- XR Lumbar Spine: unremarkable
- MRI Cervical Spine: multiple disc bulge with severe C6-7 left foraminal stenosis. Moderate degenerative disc disease.
- MRI Thoracic Spine: small right broad based right paracentral disc protusion with annular tear at T6-7.
- XR Left Ankle: Negative
- XR Left Shoulder: No acute osteoarticular abnormality

Consultations:
UF Neurology - differential including miyoshi myopathy recommending genetic testing. Genetic testing positive for homozygous ANO5 mutation consistent with Miyoshi myopathy type 3.
