Initial limited x-ray (AP, internal rotation, and lateral scapula views one week prior to visit): unremarkable.
Bedside US: no rotator cuff or biceps tear, but there is a 1.7cm x 1.5cm x 0.8 cm heterogenous mass with a hyperechoic rim and heterogenous core. There are a few hyperechoic punctate calcifications at the medial aspect of the bicipital groove Case Photo #1 . There is fluid within subcoracoid bursa and positive impingement of the bursa with dynamic maneuvers.
MRI: intact rotator cuff and biceps tendon, benign mass at medial aspect bicipital groove suspicious for muscular tear Case Photo #2 .
Repeat x-ray: subtle calcification over the lesser tuberosity Case Photo #3 , Case Photo #4 .
