Patient Presentation:
22 y.o Division I, previously healthy, collegiate swimmer presents with complaint of "my heart feels out of whack". He reports a history of exertional chest discomfort for the last few months with increasing frequency of occurrence. He complains of palpitations and mild dizziness with swimming but denies syncope. Episodes occur with swimming but occur more frequently with weight training. He denies new medications, supplements, or drug use. He admits to heavy alcohol use on the weekends but symptoms have not occurred while drinking.

History:
Past Medical- Negative
Family - Negative
Social - Occasional alcohol, no drug or tobacco use

Physical Exam:
Blood Pressure 112/78, pulse 74, Respiratory rate 12, weight 202 lbs.
General - No acute distress
Pulmonary - Clear to auscultation, no rales, ronchi, or wheeze, bilateral equal breath sounds
Cardiology - S1/S2 normal, RRR, no rubs murmurs or gallops. No carotid bruits, pulse equal in all extremities
Further physical exam is normal
