Patient Presentation:
A 19 year old male collegiate athlete with left cheek swelling.

History:
A 19 year old Caucasian male collegiate football player presented with right cheek pain, swelling, and fever which started one day prior. Review of systems was positive for total body pains, fatigue, loss of appetite, and right ear pain. He denied rash, sore throat, nasal congestion, or vomiting. Additionally, his roommate was recently treated for streptococcus throat infection.

Physical Exam:
The vital signs were positive for an elevated temperature.
He was in no acute distress. He had visible swelling of his right cheek that was tender to palpation. The swelling was ill-defined and extended behind the angle of the mandible Case Photo #2 . There was no erythema and he had normal pain free range of motion of the mandible. Additionally he had two swollen and tender right anterior cervical lymph nodes Case Photo #1 . The rest of his head, ear, nose, and throat exam was normal. His abdominal exam elicited mild tenderness in the left upper quadrant with no palpable hepatosplenomegaly. His heart and lung exam was normal.
