Portable Ultrasound: Normal triphasic flow in the popliteal artery in PDI mode. However, with gentle active plantar flexion and passive dorsiflexion there was complete cessation of flow
MRI/MRA: Medial and lateral heads of the gastrocnemius muscles were normal with no abnormal soft tissue identified in between arteries or veins. Case Photo #1 Case Photo #2 Case Photo #3 No focal occlusion or high-grade stenosis was identified.
Compartment testing: Mildly elevated but not diagnostic
Consultations:
Given inconsistent findings for popliteal artery entrapment syndrome, patient was referred for additional work-up and evaluation. ABI testing was positive for claudication with provocation
Right ABI: Rest 1.03, Plantar flexion 0.69, Dorsiflexion 0.79
Left ABI: Rest 1.01, Planter flexion 0.54, Dorsiflexion 0.45
